2020-2021 Enrollment Forms & Registration


This is for students brand new to LCM. 

If you have an incoming TK or kindergarten student that you need to register or know of a family that will be new to LCM next year, below is important information regarding the registration process.

Step 1: Please click appropriate grade level and fill out the Google Form to begin the registration process for the 2020-2021 school year.

Step 2: A member of the LCM enrollment team will email you an enrollment packet.

Step 3: Parents complete the packet electronically and email it back to the following enrollment email address: LCM-Enrollment@smusd.org . Please make sure to include copies of the additional documentation needed (i.e. proof of residency, copy of birth certificate).

Step 4: A member of the LCM enrollment team will process your paperwork and contact you if additional information is required.


If you obtained a paper packet prior to school closure:

If you obtained a paper enrollment packet prior to school closure, you may turn it into the LCM office on Mondays between 9:00 AM and 12:00 PM and Wednesdays between 1:00 PM and 3:00 PM.  A member of the LCM enrollment team will be available to take your packet.  Social distancing practices will be in effect and we respectfully ask that a facial covering be worn when entering the front office.
